Forms All of the application forms related to land development are listed below. 

Application Fees The Common Council has established various review fees to cover the City's administrative costs. The fee must be paid before we can begin reviewing an application.

Submittal Deadlines Some applications can be submitted at anytime. Others need to be submitted before a certain time, and determined to be complete, so they can be added to a meeting agenda. Because of that, you may want to submit your application before the deadline. That way, we can let you know if anything is missing and you'll have some time to perhaps get it done before the deadline.
 
Review Procedures Once an application is submitted, staff will review it to make sure it is complete. Once that determination has been made, it will be sent to the appropriate person or City body that has the authority to review it and render a decision. Depending on the type of application, there may be just a couple of steps in the process, or many. Check out the Procedures page, which describes each of the review procedures.

Projects Requiring Multiple Reviews  Some projects will require a number of reviews. To the extent possible, we will try to schedule such reviews concurrently.
 
Tips
  1. Depending on the nature of the application, you may need to include supplemental materials. Be sure to include them with your submittal.
  2. Please take the time to provide the information that is requested. If your application is missing needed information, it could potentially delay the review of your application.

Administrative Appeal Application

Use this form when appealing a decision of the Zoning Administrator. See Article 5 of the Zoning Code for more information.  

Annexation Application

Use this form to submit an annexation request. In addition to this form, an annexation petition and State review form are required. Please contact staff with any questions.

Architectural Review Application

Use this form when applying for an Architectural Review. See Article 5 of the Zoning Code for more information.  

Certificate of Zoning Compliance Application

Use this form when requesting a certificate of zoning compliance.  See Article 5 of the Zoning Code for more information. 

Certified Survey Map Application

Use this form for any division of land other than a subdivision within the City or its extraterritorial plat approved jurisdiction.  See Section 12.09 of the Municipal Code, the Wood County Land Subdivision Ordinance and Chapter 236, Wisconsin Statutes.

Code Amendment Application

Use this form to apply for an amendement to the text of the zoning code or for an amendment to the zoning map. See Article 5 of the zoning code for more details

Code Interpretation Application

Use this form to apply for code interpretation. See Article 5 of the zoning code for more details

Conditional Use Application

Use this form for applying for a conditional use or an amendment to one.  For more details see Chapter 5 of the Zoning Code.

Conversion of a Nonconforming Use Application

Use this form when requesting the conversion of an existing nonconforming use. See Article 5 of the Zoning Code for more information.

Expansion of a Nonconforming Building Application

Use this form for applying for the expansion of a nonconforming building.  See Article 5 of the Zoning Code for more information.

Floodplain Permit Application

Use this form when applying for any type of work or improvement within the Floodplain.  See Article 5 of the Zoning Code for more details.

Home Occupation Application

Use this form when looking for approval on a home based business.  See Article 5 of the Zoning Code for more information.

Plan of Operation Application

Use this form when requesting a Plan of Operation review.  See Article 5 of the Zoning Code for more information. 

Planned Development District Application

Use this form when a Planned Development District is needed.  See Article 5 of the Zoning Code for more information.

Registration of a Nonconforming Use Application

Use this application when looking to register an existing nonconforming use.  See Article 5 of the Zoning Code for more information.

Use this form when making a setback averaging request. See Article 6 of the Zoning Code for more information.

Sign Exception Application

Use this form when applying for sign exception requests where compliance with Chapter 46 (Sign Code) is not met.  See Section 46.25 of the Sign Code.

Termination of Approval Application

Use this form when there is a need to remove a previously permitted or conditional use from a property. See Article 5 of the Zoning Code for more information.

Use this form when applying for a new telecommunication facility, a Class 1 collocation, or a Class 2 collocation.  See Article 5 of the Zoning Code for more information.
